Do Your Homework

All doctors have paperwork. Do ours ahead of time at your convenience by downloading it here. Complete it and bring it with you. This keeps your appointment time as short as possible.

Come on In

Come to your appointment. Bring any pertinent, easily available imaging, tests or reports with you. If you can’t get your hands on that stuff before your appointment, don’t worry about it for now.
